INFORMATION ON THE PROCESSING OF PERSONAL DATAŮ

in accordance s Atřízením Europeanéhim in parliament a Council Regulation (EU) 2016/679 from on 27. April 2016 O protectioně Physicalýof persons in connection with with processing of personal dataů a free movement těchto úgiveů and about cancellation of směrticle 95/46/EC (dájust "Tořízeni")

 I am informed that in connection with my členstvím in Boxer Pro Gym

("Administrator"),

is obliged to process my Personal Data on the basis of §3a (3) of Act 115/2001 Coll. on the Promotion of Sport (hereinafter referred to as "ZOPS"):

1. Name, surname, title.
2. Date of birth.
3. Address of the place of residence.
4. Contact details including telephone and e-mail.
5. In the case of a foreigner, also: the address of the place where he/she is mainly staying, his/her nationality and the number of his/her identification document (passport).

All personal data is processed for the purpose of keeping records of the membership base and related activities.

I have been informed that on the basis of §3a (6) of the Personal Data Protection Act, the Personal Data pursuant to points 1) to 4) will be processed and stored for a period of 10 years from the date on which I cease to perform the activity for which I am registered by the Administrator.

I am informed that, based on the legitimate interest of the Administrator, Personal Data for the purpose of keeping records of the membership base and related activities will be processed and stored for a period of 10 years from the date when I stop performing the activity for which I am registered by the Administrator.

I acknowledge that the Controller stores data for the sole purpose of

1. keeping records of the Boutique Gym membership base
2. in case of consent, sending mass mailings to interested parties (newsletter).

I am informed that if I refuse to provide the aforementioned Personal Data, I cannot become a member of the aforementioned fitness club.

CONSENT TO THE PROCESSING OF PERSONAL DATAŮ

I consent to the processing of my personal data by the Controller (the aforementioned club) on the basis of Article 6(1)(a) of the Regulation.

2.  e-mail,
3. date of birth.

for the purpose of keeping records of the membership base and related activities (e.g. applications for state subsidies, insurance processing, etc.)

I agree that my Personal Data will be processed and stored for the entire duration of the purpose of processing (i.e. even after I stop performing the activity for which I am registered by the Controller).

I understand my right:

• to have access to your Personal Data (pursuant to No. 15 of the Regulation),
• demand their correction (according to No. 16 of the Regulation),
• to delete the Personal Data without undue delay, if the reasons under No. 17 of the Regulation are given,
• to restrict the processing of Personal Data in cases pursuant to No. 18 of the Regulation,
• to data portability in the cases provided for in No 20 of the Regulation,
• withdraw this Consent pursuant to No. 7(3) of the Regulation,
• file a complaint against the Data Controller pursuant to No. 77 of the Regulation.

I declare that I have carefully read the text of this consent, understand and agree to its contents. This is confirmed by my handwritten signature.
